Information Form

To begin the purchasing process, please fill-out the information form below. When you click submit, you will be redirected to a new webpage that will allow you to select the amount you wish to pay for your streaming pass ($15, $25, or $50) and complete the payment. The LBMT Box Office (lostbaggageboxoffice@gmail.com) will email you the streaming pass within 24 hours, and you will have one week to watch the show as many times as you like.

PLEASE NOTE: Filling out the information form does not guarantee you access to the show nor does it reserve you a streaming pass. You must submit the information form AND complete the payment to receive a streaming pass. You will be redirected to our secure payment processor (administered through PayPal or Square) upon submitting the information form.
